Cognitive Support and Dementia Nurse Consultant
Some people with cognitive impairment or dementia have behavioural or mood problems. Examples of these include low mood, agitation, frustration or irritability, calling out and wandering, but there are many more. These are known are Behavioural and Psychological Symptoms of Dementia (BPSD).
Cognitive support and behavior management includes a person-centred assessment involving people experiencing BPSD symptoms and their family members. It includes education, support, and creating a care plan or easy guides to manage challenging behaviours. This helps meet the needs of people with cognitive impairment or dementia.
Clinical Nurse Consultant
Antonella is a Clinical Nurse Consultant who provides Cognitive Support and Dementia Support. She is dedicated to helping people with cognitive issues like dementia and delirium. She works closely with geriatricians, GPs, and other services to provide expert nursing advice, comprehensive care plans, and offer management recommendations.
Antonella has over 25 years of nursing experience in acute, sub-acute, and community hospital programs, and in aged care. She works in both public and private health sectors. She provides personalised, compassionate, and holistic care with a practical, hands-on approach.

Request nursing services
If you are a patient/family/carer that would like to access the service please complete CS&D Nurse Consultant - Assessment Request form and return it to Aria Health.
GP referrals
If you are a GP that would like to make a referral, please send a standard referral to Aria Health requesting “Cognitive Support and Dementia Nurse Consultant assessment” with appropriate contact details for your patient. We will then make contact with patient/family/carer to give them more information about the service.
